Case Manager Palmetto Pee Dee Behavioral Health is a 59-bed behavioral health facility located in Florence, SC offering adolescent behavioral health programs and treatment services to males and females ages 7-21.
The Case Manager is responsible for providing Case Management services and ongoing communication, evaluations, and assessments to youth, families, and Placement Agencies under the direct supervision of the Clinical Director.
Complete or arrange for all required mental health/behavioral health screenings and assessments.
Complete performance summaries, treatment plan reviews, aftercare/transition plans, and discharge summaries.
Assist Medicaid Clerk or other staff in post release data collection as required by Palmetto Pee Dee policy and contractual obligations.
Maintain all case management files and paperwork per Palmetto Pee Dee policy.
Serve as liaison between the program, the contracted agency, other outside agencies, and/or the youth's family.
Conduct intake process for all new clients.
Provide case management duties, preparing families/guardians/agencies for program visits, home leave experiences, conferences, and aftercare responsibilities.
Network family members to available community resources.
Facilitate patient contact with guardian/agencies by maintaining supervision of incoming/outgoing mail and assisting with phone calls.
Collaborate with leadership and performance improvement activities by conductingperiodic reviews to ensure compliance with Palmetto Pee Dee policy and agency requirements.
Maintain a high standard of role model behavior for youth.
Participate in training and development activities as required.
Report any acts, incidents or conditions that reflect the possibility of inappropriate youth-to-youth or staff-to-youth relationships.
Report any use of physical force and all unusual incidents per Palmetto Pee Dee and DHEC guidelines
Perform other duties as assigned or needed.
